Confirm that I have read, understood and will abide by the TOP Dogs Procedures and Disciplines.

• I also understand that if I don’t abide by these conditions I could be requested to discontinue visits in the name of ‘TOP Dogs’, and my membership was withdrawn.

• I will wear an appropriate uniform with a TOP Dogs badge, and make sure my dog is clean and healthy prior to visits.

• I will also endeavour to attend my visits on a regular basis, so as not to cause disappointment to those we visit.

Having agreed to all of the above I request to become a member of TOP Dogs.
